Budget Request Form

We are excited for you as you move forward in creating your Reunion/Shabbat Dinner Event. Please fill out this form to submit your food and beverage budget request. We are prepared to cover up to $10/person including tax. Please submit this at least 5 business days before your event for allocation.

When filling out the form below, be sure to use the same Payment ID from the home page on your portal. Copy and paste the ID from HERE to ensure no errors. Any error in your Payment ID will result in a delay in processing your request. The format for the Payment ID is P######

Passages Staff Name:

All requests will need to be verified by a Passages Full-Time Staff member before they are processed. Make sure to select the staff with whom you have been communicating with about the expense. If you don’t know which Passages Staff will need to approve your request, you can default to Nancy Sparks. She won’t be approving it but rather reaching out to the Passages Team to find the responsible party. It will cause a delay in your request.

For many requests, the following staff will be the responsible party. They will be notified once you finish the form and will either approve or deny it. If they aren’t the one you were communicating with, it will delay your request as they try and find the right person.
