Bento Order Form

Ambassadors will be receiving a Bento MasterCard, which is a Corporate Pre-Paid credit card. Passages Staff will be able to see each transaction in real-time and will be tracking your usage and will make adjustments as needed. Once you receive your card, please activate it and download the “Bento for Business App” on your phone or access your account online to track all of your activity. If you have any problems activating your card or using the app, please let us know. Keep this card for future events you host as an Ambassador this semester. Passages can replenish the amount on your card for future needs.

Once your card is ordered, please refer to the Budget Request Form to request the amount you need to be allocated onto your card. The card will take approximately 2-3 weeks to arrive, so we encourage you to plan accordingly.

When filling out the form below, be sure to use the same Payment ID as provided in emails from Passages. Copy and paste the ID as provided to ensure no errors. Any error in your Payment ID will result in a delay in processing your request. The format for the Payment ID is P######

Passages Staff Name:

All requests will need to be verified by a Passages Full-Time Staff member before they are processed. Make sure to select the staff with whom you have been communicating with about the expense. If you don’t know which Passages Staff will need to approve your request, you can default to Nancy Sparks. She won’t be approving it but rather reaching out to the Passages Team to find the responsible party. It will cause a delay in your request.

For many requests, the following staff will be the responsible party. They will be notified once you finish the form and will either approve or deny it. If they aren’t the one you were communicating with, it will delay your request as they try and find the right person.
